testicular sperm extraction was carried out on the 25 th of february 2010 under general anesthesia though a om all vertical incision in the median scrotal Raphe (2cm), the skin , the dartos muscle, and tunica vaginalis were opened to expose the tunica albuginea. the aubtunical vessels under the surgical microscope were i identified and avoided. a stay suture of 5/0 prolene was placed into the tunica albuginea and then linear transverse (1cm) incision was done. the testicular tissues of the right testis were observed under optical magnification (x24) but no morphologically dilated tubules could be observed, the incision was extended and blunt dissection performed between the septa of the testicular parenchyma to expose multiple areas. copious irrigation of the field with phosphate buffered saline solution was carried out to prevent blood from obscuring the field, and the most dilated tubule was measured using a micrometer fixed to one eyepiece of the operating microscope. this tubule was further dissected and isolated from the adjacent tissues and examined for the presence of spermatozoa. bipolar diathermy was applied carefully to ensure proper hemostasis. another sample was taken from the second most suspicious tubules after measuring its diameter. however, no sperm cells could be identified therefore, 2 samples were harvested from 2 different sites and tissue were minced and examined under the inverted microscope, but no sperm cells were observed. the left testis was the left testis was delivered and the same s were followed. testicular tissue samples were surgically examined for the presence of spermatozoa but no sperm cells were identified. the tunica albuginea was closed with prolene 6/0, and the tunica vaginalis, dartos muscle and skin were closed using 4/0 vil in separate layers. the seminiferous tubules were minced in a petri dish containing hepes buffered media and checked for spermatozoa under the inverted microscope but no sperm cells were identified even after centrifugation and erythrocyte - lysing buffer(elb) analysis.IVF laboratory findings: sertoli cells, spermatogenic cells including primary spermatocytes only. follow up in the clinic was carried out and the wound healing was non-remarkable. this report is done upon patients request.
